biography

Jake completed a combined MD and MPH program at Tufts University in 2010 before heading west for his internal medicine residency training at the University of Washington. His career has focused on practicing and teaching primary care, health policy, health systems, and healthcare leadership. Jake's interests have spanned multiple domains and roles, from clinical practice leadership as medical director at the UWMC – Roosevelt General Internal Medicine Center, to serving as an associate medical director with a health plan, to leading primary care innovation initiatives with UW Medicine’s care transformation team, to advocacy and policy analysis with the Society of General Internal Medicine’s health policy committee. He now serves as the Medical Director for Population Health Integration for UW Medicine, working across the health system to expand, strengthen, and coordinate population health and value-based care efforts beyond the UW Medicine primary care network. Jake’s core interest is working at the intersection of policy and clinical practice, seeking to strengthen alignment between payment models, population health goals, patient-centered care, and support for the healthcare workforce. Beyond the healthcare sector, Jake has enduring interests in geopolitics and American history and enjoys the outdoors, travel, movies, and, above all, time with his family, including his two young daughters.
